If you have any questions regarding the processing of your personal data at FSHS, please contact the persons responsible for data protection by using the form below. The persons responsible for data protection at the FSHS are Marjo Tipuri and Aleksi Schrey. You can also contact the person responsible for data protection by phone on Fridays at 10-11 am, + 358 40 577 5526.
If your matter concerns the ordering of patient records, please select the form Request for copies of patient documents.

- Before filling in the form, you should be registered in the suomi.fi-service. In case you have not registered yourself then go to https://www.suomi.fi/frontpage and from there to the `identification´ section.
- In case you have registered yourself before then you can proceed by filling in the form.
- When you are submitting the form you will be identified through the suomi.fi –service either through using your online banking codes or your Mobile ID.
- Remember to give the permission to be contacted through the suomi.fi –service. In this case we can respond to your feedback. We answer to all feedback, which has been made after identification, within three working days.
