Request for copies of patient documents

  • Before filling in the form, you should be registered in the suomi.fi-service. In case you have not registered yourself then go to https://www.suomi.fi/frontpage and from there to the `identification´ section.
  • In case you have registered yourself before then you can proceed by filling in the form.
  • When you are submitting the form you will be identified through the suomi.fi –service either through using your online banking codes or your Mobile ID.
  • Remember to give the permission to be contacted through the suomi.fi –service. In this case we can respond to your feedback. We answer to all feedback, which has been made after identification, within three working days.
  • Please note that you cannot order both laboratory tests and imaging results with the same form. If you need both, please fill in a separate request for both.
