How to order your patient data

Patients can ask for copies of their medical records either by sending us a free-format letter or using the pre-prepared request form available from our health centres. The request will be processed by the person responsible for the sector within around one week of the submission of the request.

X-ray and ultrasound images are available on loan if necessary. The images must be returned to the same place.

Dental images can be copied onto a CD.

Please include the following details in your request:

• name
• personal identity code
• address and phone number
• the sector from which you would like to order documents (general health, oral health, mental health)
• which information you want and for which period of time

Remember to sign and date your request.
