Request Patient Data from FSHS Joensuu

You may request your patient data using a free-form application which includes the following information:

• name (including your previous names), personal ID
• addres and telephone number
• the data requested and the perioid concerned.

The request must contain your personal signature, your name in block letters, the place and the date.

Your data will be available for you to pick up at the unit, or, if so agreed, we can send it mail, in about one week.
